PHP Form - implement working form script in new website.

| Apibūdinimas: | Basically, the form script that exists at arrestattorney.com/aaindex18.html is the one I want at the bottom of arrestattorney.com/muse/index7.html. Right now, the obstacles I am facing are as follows: First, I couldn't get the validation rules to transfer from the first to the second website. i.e. in the second website, it won't stop you from sending the form with only one field filled out. So if you could transfer the validation rules from the first, that would be great. Second, when you click, Court Date, in the first one, a calendar pops up allowing you to choose a date. Not so in the second, which I want to fix. Third, when you put in content, and then refresh the page, the content you put in should revert back to the original placeholder, But it doesn't. The content remains in the field after a refresh. Finally, when the form is submitted, it takes it to another page. I would like it to do what it does on the other page when it successfully submits, i.e. something pops up to confirm the message was sent. I'll give you five for each if you can do it. I'll attach all the files from each.
